A

Trachea away from white out
- Pleural effusion
- Diaphragmatic hernia
- Large pulmonary mass

Trachea midline
- Consolidation
- Pleural effusion/ARDs
- Mesothelioma
- Chest wall mass

Trachea pulled towards
- Pneumonectomy
- Total lung collapse
- Pulmonary agenesis

Pneumothorax Mangement

A

Traumatic - ICC insertion
Tension - urgent needle decompression

Conservative (small <50% hemithorax, no symptoms)
- Observation, repeat CXR

Active (large >50% hemithorax)
- Aspiration OR
- ICC Insertion

Indications for ICC > Aspiration

A
  • Failed aspiration
  • Secondary spontaneous pneumothorax - unless the patient is stable
  • Traumatic pneumothorax - unless patient is stable
  • Decompensated pneumothorax
