starts wrists/ankles, then spreads (including palms/soles)
initially discrete, macular lesions, becomes petechial
RMSF
keratoderma blenorrhagicum seen in secondary syphilis (also in reactive arthritis)
lues maligna lesions (rare presentation of secondary syphilis, mainly in HIV)
dimorphic fungi w/ lesions on facial skin around mouth/nose
mucocutaneous paracoccidiomycosis
dimorphic fungi that have skin lesions in 70% of persons
papules on face, chest, extremities
may umbilicate - resemble mollscum
consider talaromyces
image of palm w/ target lesion next to herpes labials on patient’s lips
consider HSV-1 that is a/w recurrent erythema multiforme
eczema herpeticum
HSV spreading to skin that is already compromised by pre-existing skin disease
new painful lesions on pre-existing dermatoses
cutaneous larva migrans d/t ancylostoma braziliense larvae (dog hook worm)
serpiginous, “creeping” rash on extremities
very itchy, rarely painful
rapid, migratory erythematous rash
any part of body
often seen perianally
consider strongy
perianal d/t passage of larvae in the stool
dx via stool studies
tx w/ ivermectin
intermittent swelling w/ painful, red areas of swelling/induration
migrate q few wks
consider gnathostomiasis
ddx for umbilical papules
- cutaneous crypto
- histo
- molluscum
- talaromyces
ddx for infection-related lesions on palms and soles
- EM (recurrent, triggered by HSV-1 outbreak)
- HFM (CV A16, EV71)
- osler or janeway lesions a/w IE
- N gonorrhea (w/ disseminated infection)
- N meningitidis (w/ meningococcemia)
- RMSF
- syphilis
ddx for ascending lymphangitis
AKA “sporotrichoid spread”
- cutaneous leishmaniasis
- M marinum
- nocardiosis
- sporothrix
- systemic mycosis sometimes (cocci, blasto, histo)
