Pemphigus vulgaris
IgG antibody against desmoglein
IF = reticular antibodies around epidermal cells
+ nikolsky sign (separation of epidermis upon manual stroking of skin)
bullous pemphigoid
IgG against hemidesmosomes = (bullow the epidermis)
IF = linear pattern at epidermal-dermal junction
dermatitis herpetiformis
IgA deposition at tips of dermal papillae –> associated w/ celiac disease
erythema multiform asociations
1) mycoplasma pneumonaie / HSV
2) drugs = sulfa drugs, B-lactams, phenytoin
3) cancers / autoimmune
Stevens Johnson syndrome
1) fever
2) bulla formation / necrosis
3) sloughing of skin
4) high mortality rate
can evolve to toxic epidermal necrolysis
erythema nodosum
painful, inflammatory lesions of subcutaneous fat –> on anterior shins
-idiopathic, associated w/ sarcoidosis, coccidiodomycosis, histoplasmosis, TB, streptococcal infections, leprosy, crohn disease
lichen planus
1) pruritic
2) purple
3) polygonal planar papules / plaques
4) associated w/ hep C
lichen planus on histo
sawtooth infilitrate of lymphocytes at dermal-epidermal junction, associated w/ Hep C
basal cell carcinoma
1) rolled borders
2) central crusting / ulceration
3) seen as “palisading nuclei” on histology
squamous cell carcinoma
2) ulcerative lesions w/ frequent scale
2) chronic draining sinuses
3) keratin pearls on histology
mlanoma
S100 tumor marker
