Outline the pressure injury stages

A

Stage I: Non-blanchable redness of intact skin; may feel warmer or cooler.

Stage II: Partial-thickness loss with a shallow ulcer or blister.

Stage III: Full-thickness loss extending into subcutaneous tissue; fat may be visible.

Stage IV: Full-thickness loss with exposure of muscle, bone, or supporting structures; may have slough or eschar.

Unstageable: Full-thickness loss with slough or eschar covering the wound, preventing depth assessment.

Deep Tissue Pressure Injury: Discolored area with intact or non-intact skin; may be painful or firm.
