What 7 things do you do when you first come into the patients room?

- Check the MAR to the physicians order
- Wash hands
- Check patients identity
- Ask about allergies and their reaction
- Ask if they have any pain, use scale of 1-10
- Go through the medications with them
- Wash hands and exit room to go call physicain

When speaking to the physician, what do you do if he gives you a new order?
A
Write it down and repeat it back to the physician to make sure it is correct.

What do you do upon entering the room of the patient after speaking to the physician?
A
- Raise bed and Wash hands
- Check the patients identity.
- Put on gloves
- Go through each med with patient and identify what each medication is for.
- Administer each medication
- Lower bed and put up rails.
- wash hands and exit room

Otic?
A
- Warm the solution
- Gently shake the bottle
- Put patient in Simms position
- Cleanse the external ear with a cotton tipped applicator
- Pull the pinna up and back.
- Shake the pinna in between drops and when done.
- Tell patient to remain on their side for 5-10 minutes.
- Place cotton loosely in the ear.

Opthalmic drops?
A
- Cleanse the edges of the eyelid from the inner to the outer canthus.
- Holding eye dropper, rest dominant hand on forehead
- Pull down the lower eyelid with the non-dominant hand
- Ask patient to look up
- Place drops in the conjunctiva sac.
- Ask the patient to close eyes and move eyes around
- press gently against the side of the nose.

A
- Have patient rinse mouth out.
- Explain how its going to work
- administer med
- have patient rinse mouth out and spit out the water.

With an inhaler, how long do you wait in between puffs?
A
1 minutes.

Rectal?
A
- Ask patient if they need to defecate
- Put in Simms position
- Put on gloves
- Remove suppository wrapper and lubricate the smooth end and the tip of the gloved finger.
- Say “you will feel some pressure.”
- Use nondominate hand to separate cheeks.
- Ask patient to take a deep breaths in and out.
- Push it in about 3 inches. (0.5-1 inch in infants)
- Wipe the anus clean with a toilet tissue
- Ask patient to stay on side for 5-10 minutes.
- Ask them to hold in for 30 minutes.
- Leave bedpan within reach if it was a suppository.

Vaginal?
A
- Ask if patient needs to void
- Put them into the Simms position
- Don gloves
- Remove wrapper from suppository and place it loosely in wrapper container.
- Clean the vigina
- Separate labia with non-dominate hand.
- Tell them they may feel some pressure
- Push in the suppository about 3 inches.
- Ask the patient to stay on their side for 5-15 minutes.
