what is sjogrens
autoimmune condition that effects the exocrine glands - characterised by lymphocytic infiltrates
pathophysiology of sjogrens
immune system mainly attacks lacrimal and salivary glands
symptoms of sjogrens
dry eyes - gritty feeling
dry mouth - may lead to fissured tounge
dry throat
vaginal dryness
joint pains
fatigue
signs of sjogrens
bilateral parotid gland enlargement
unexplained increase in dental caries
Ix in suspected sjogrens
schirmers test = ocular dryness
postive anti-Ro and anti-La antibodies
lymphocytic infiltrates on lip gland biopsy
describe schirmers test
paper strip inserted into the corners of the eyes for several minutes to measure tear production
Mx of sjogrens
symptom control - tear and salivary replacement, analgesia, exercise
hydroxychloroquine can sometimes help with arthralgia and fatigue
immunosuppression would only be used in the context of organ involvement e.g. interstitial lung disease
complications of sjogrens
inreased risk of lymphoma
peripheral neuropathy and interstitial disease
sjogrens less commonly has other organ complictaions so no monitoring
