Sisson Injectables Flashcards
What did the DCCT/EDIC trials establish?
The treatment of microvascular disease early on in life is more beneficial due to metabolic memory
What are the main effects of GLP-1?
- Enhance glucose-dependent insulin secretion
- decreases postprandial glucagon secretion
What did the EUREXA trial establish?
Exenatide attained A1c of 7% and 6.5% better than glimempiride.
Also, exenatide showed signs of weight loss
What does the structure of liraglutide look like?
GLP-1 structure but with a fatty acid chain attached
What is Exenatide LAR?
Bydureon which is an extended release version of Exenatide
What are side effects of GLP-1 agonists?
Delayed gastric emptying
Hypoglycemia
Macrovascular risks
Headache, N/V
GLP-1 agonists has not been proven to cause pancreatitis. T or F
True
Diabetes increases risk 3 fold
Most common causes of pancreatitis are alcohol and gallstones
Patients taking liraglutide should be concerned about which ADR?
Potential for development of Thyroid C-cell cancer (not really established in humans but rodent and monkey data show signs)
What is a major ADR associated with Exenatide?
Renal failure
What is Pramlintide Acetate? Which device is it used in? Why use this medication?
What does amylin do?
Pramlintide acetate is an analog of amylin. It is used in Symlin. Amylin secretion follows the pattern of insulin however, amylin secretion is absent during diabetes.
- Amylin slows gastric emptying
- suppresses glucagon
- promotes sateity
What differences do you see when administering Symlin in TDM1 vs TDM2 patients?
In TDM1, there is more likelyhood that N/V symptoms occur because of many escalations of dosings as opposed to dosing in TDM2
