Q

PROS and CONS of an elective induction of labor

A

pros -
control of when labor starts
can plan for support people to be there
will be in hospital so more safe
can avoid macrosomia

cons -
risk of low bishop score
risk of c/s
baby may have some difficulty adjusting to induction, requires UNCOIL

Q

example:
dilation 1 cm = 1
effacement 30% = 0
station -3 = 0
soft cervix = 2
midposition cervix = 1

bishop score = 4

what does it mean?

A

This is a low score. Ideal score is 8 +

This means a vaginal birth is unlikely.

Labor augmentation would be a good thing to do, such as foley bulb or prostaglandins.

Q

is this strip reassuring?
baseline -
variability -
accelerations -
decelerations -
cause of decels -
contraction frequency -
contraction duration -
contraction intensity -
contraction resting tone -

any interventions necsessary at this time?

A

reassuring - yes
baseline - 150’s
variability - minimal
accelerations - none (nonreactive)
decelerations - early
cause - head compression
contraction frequency - 120 - 180 secs
contraction duration - 60- 90 secs
contraction intensity - must palpate
contraction resting tone - resting tone visible between contractions 30 - 90 secs

the baby’s head is expected to be depressed as it moves down the birth canal. we may want to do a SVE and we could still reposition or do other nursing interventions.

category 1, 2, 3 -

any interventions necessary at this time?

A

reassuring - yes
baseline - 145’s
variability - moderate
accel - yes (reactive)
decels - no
contractions - none
category 1 - good

we could increase oxytocin since mom is not having any contractions and FHR is reassuring
