Shoulder OSCE Flashcards
ROM for arm
Flexion- 180 Extension- 60 Abduction- 180 Arm horizontal adduction- 130-140 Arm horizontal abduction- 130-145 internal and external rotation- 90
Glenohumeral instability tests
- Apprenhension tests- abduct and flex shoulder to 90. force arm into external rotation
- Sulcus sign
Yergason’s test
Flex arm to 90. Pt hand is supinated and docs hand grabs wrist.
have pt supinate and ER against resistance.
–> unstable biceptial
Speeds test
flex pts arm to 50-90 and flex elbow.
Tell pt to flex shoulder while you resist
+ pain in biceptial groove
indicates biceptial tendonitis of longhead biceps
What tests test rotator cuff part
- Empty can (esp supraspinatus)
2. Drop arm test (full tear of supraspinatus)
empty can test
Rotator cuff pathology- esp supraspinatus
Drop arm test
pt abducts arm to 90, then slowly drops arm
+: arm will drop
Indicates: full thickness tear of supraspinatus
Painful arc test
+ pain between 60 and 120 degrees of shoulder abduction
Indicates: rotator cuff injury
Neer impingement
Stabilize the patient’s shoulder, with the patient’s forearm pronated, passively flex the
shoulder into a fully flexed position
+: pain
indicates: subacromial bursa or rotator cuff impingement
Hawkind test
Flex the shoulder and elbow to 90 degrees, passively rotate the humerus into internal
rotation
+ Pain
Indicates: rotator cuff or subacromial bursa impingement
Rotator cuff impingement tests
- painful arc
- Neer impingement
- hawkins test
Subscapularis test
lift off tests
+ inability to resist (weakness)
AC tests
Cross arm test- doc passivly adbucts patients arm across chest
+: pain in AC joint with end range adduction
Indicates: AC joint pathology
Test to check ROM
appley scratch test
