16
Q

scarf test

A

pain - ACJ pathology/injury

17
Q

shoulder - joint above

A

C spine FERL

18
Q

shoulder - neurovascular

A

power - deltoid (axillary)
sensation - regimental badge (axillary)
vascular - radial

19
Q

shoulder - feel for

A

tenderness, bony abnormality (eg osteophytes, steps)

20
Q

shoulder - radial, median, ulnar sensation

A

radial - dorsal 1st interosseus space
median - lateral index finger
ulnar - medial little finger

21
Q

shoulder - i would also

A

above + below (elbow + c spine), then MSK
xrays (modified + axillary views)
MRI
joint aspiration

22
Q

impingement - what is it + tests

A

supraspinatus tendinitis (painful arc syndrome)

1 - mid arc - pain abducting 60-120 (due to impingement of underside of acromion on inflamed tendon)
2 - hawkin’s kennedy test

[supraspinatus abducts arm]
no restriction of passive movement

23
Q

complications of anterior dislocation of shoulder

A
nerve damage - axillary, brachial plexus
artery damage - axillary
rotator cuff injury
associated fracture - humeral head, clavicle, acromion
recurrent dislocation

[nerve - artery - muscle - bone - joint]

24
Q

frozen shoulder - what is it, risk factors

A

adhesive capsulitis of glenohumeral joint

spontaneous, rotator cuff injury, immobility, diabetes, thyroid

25
Q

frozen shoulder - phases

A

freezing - gradual onset pain + stiffness, 2-9mo
frozen - pain goes, stiffness stays, limited external rotation, 4-12mo
thawing - gradual return of movement, 1-3y

restriction of all movements but external rotation most obv

26
Q

if pain is present on active but not passive movement what does it suggest?

A

muscular problem

27
Q

rotator cuff muscles - roles

A

infraspinatus - external rotation, with teres minor
supraspinatus - abduction
subscapularis - internal rotation

28
Q

ACJ pathology/arthritis - features OE

A

painful high arc
tender ACJ
pain on scarf test

29
Q

movements to test supraspinatus

A

thumbs up/down

30
Q

movement to test ACJ

A

touch opposite shoulder

31
Q

movement to test infraspinatus

A

touch occiput (with teres minor)

32
Q

movements to test subscapularis

A

touch opposite hip

touch ipsilateral buttock
