Give origin insertion and nerve for Trapezius
Origin: ext. occipital protuberance, medial portion of superior nuchal line of OP, ligament nuchea and spinous processes of C7- T12
Insertion: lateral one third of clavicle, acromion and spine of scapula.
N: Spinal portion of cranial nerve XI (accessory) and ventral ramus C2, 3, 4
Give action for descending(upper) trap fibers.
Upper fibers: bilaterally extend the head and neck
Unilaterally laterally flex the head and neck to the same side
Rotate the head and neck to opposite side
Elevate the scapula
Upwardly rotate the scapula
Give action for ascending (lower) trap fibers.
Scapular depression and upward rotation
Give action for middle trap fibers
Scapular retraction (adduct) / stabilize scapula
Name origin, insertion actions and nerve for deltoid
All: abduct the shoulder
Anterior: horizontally adduct, medially rotate and flex the shoulder
Posterior: horizontally abduct, laterally rotate and extend the shoulder
Origin: lateral 1/3 of clavicle, acromion and spine of scapula
Insertion: deltoid tuberosity
N: axillary C5,6
Give action for posterior deltoid fibers.
Extend shoulder
Laterally rotates shoulder
Horizontally abducts the shoulder
Give actions for middle deltoid fibers
Abducts shoulder
Give actions for anterior deltoid fibers

O: infraspinatus fossa of scapula
I: greater tubercle of the humerus
N: Suprascapular C(4), 5, 6
Origin, insertion and nerve for Teres minor
O: 2 upper thirds of lateral border of scapula
I: greater tubercle if humerus
N: Axillary C5, 6
Origin insertion action and nerve for subscapularis
A: Medially rotate and stabilize head of humerous in glenoid cavity
O: sub-scapular fossa of the scapula
I: lesser tubercle if the humerus
N Upper and lower subscapular C5, 6, 7
Action for subscap
Medially rotates the shoulder and stabilized head of the humerus
Action for infraspinatus
Lateral rotation of shoulder
Adducts the shoulder
Stabilizes head of humerus in glenoid cavity
