A

Circulatory failure resulting in inadequate tissue perfusion

Cause

A

Inadequate CO/ hypovolaemic
- bleeding or fluid loss (vomiting, burns)

Loss of systemic vascular resistence -SVR (peripheral circulatory failure) pumping fine but not going to the right places.
- Septic shock, anaphylaxis and neurogenic are all types of distributive shock

A

systemic inflammatory response syndrome (SIRS) in the presence of infection

Infection causes acute vasodilation from inflammatory cytokines

A

Acute respiratory distress syndrome

Features of ARDS

A

impaired oxygenation
bilateral pulmonary infiltrates
no cardiac failure

Causes of ARDS

A

extra pulmonary: shock, head injury, drug reaction, sepsis

Pulmonary: pneumonia, smoke inhalation, near drowning

What is auto transfusion?

A

reduced capillary BP

  • net movement of extravascular fluid back into vascular compartment.

A

exudative phase
proliferative phase
fibrotic phase

Tx of septic shock

A

ideally take cultures before antibiotics
give antibiotics within 1st hour.

e.g. tazocin + gentamicin + vancomycin
