ANA is typically assessed by
anti nuclear antibodies
assessed by direct immunofluorescence using Hep 2 cells with a technician and a microscope
where ANA+ive in patient with suggestive clinical picture
reasonable to measure ENA and anti-DNA levels which have greater specificity and prognostic value
complement levels that should be checked
typically measure C3 and C4
levels will be low in active lupus and correlate with activity of the disease
level may be high in inflammatory disease
levels may be low in inherited disorders of complement deficiency
C3 and C4 levels may be high in
inflammatory disease e.g RA
C3 and C4 levels may be low in
active lupus (correlates with activity of disease)
may be low in genetic disorders of complement deficiency eg. C4 null alleles associated with autoimmune disease

5% of normal population have +ive RF
high titre RF has some prognostic significance
ANCAs
anti-neutrophil cytoplasmic antibodies
tissue phase antibodies
have an association with vasculitis
positive predictive value
the likelihood of an individual with a positive test having the disease
negative predictive value
the likelihood of an individual with a negative test not having the disease
anti-Ds DNA is associated with
SLE
anti-Sm his associated with
SLE
anti-Ro/La is associated with
SLE, sjogrens, neonatal lupus
anti-RNP is associated with
MCTD, SLE
Anti-Scl70 is associated with
systemic sclerosis
