A

A

Increase observations to 4 hourly or more

Monitor urine output and record observations chart

What is NEWS score 5 or more? (medium ) or score 3 in one of the physiological parameters

A

Registered nurse must assess the patient, increase observations to 2 hrly for 6 hours minimum
Check blood glucose, contact FY2 and reg for review
alert NIC
strict fluid balance
screen for severe sepsis

A

inform nurse, must immediately asses and request immediate reg review
Increase obsto 1 hrly for 6 hours minimum
reg to discuss with consultant
contract critical care outreach team
