What is septic arthritis?
Why is it a serious condition?
A
Serious infection of joint space
Can lead to bone destruction

How are joints usually infected in septic arthritis?
A
Usually results from haematogenous spread
May also follow puncture wound/skin lesion infection.

How can osteomyelitis be associated with septic arthritis?
A
In young children, osteomyelitis may spread into joint where capsule inserts below epiphyseal growth plate.

How does septic arthritis usually present?
A
- Erythematous, warm, acutely tender joint
- Decreased range of movement
- Acutely unwell, febrile child
- Often hold limb still (pseudoparesis, pseudoparalysis) and cry if it is moved
- Joint effusion may be detectable in peripheral joints
• Initial presentation may be with limp or pain referred to knee.

How is septic arthritis investigated?
A
- FBC: Increased WCC and acute-phase reactants
- Blood cultures must be taken
- Aspiration of joint space under ultrasound guidance for organisms and culture is definitive investigation.

What is treatment for septic arthritis?
A
Prolonged course of IV antibiotics
