Discuss resuscitation in sepsis

A

Should be early and aggresive once organ dysfunction has been identified

  • -Adeqaute fluid loading, estimated by CVP, Pulse pressure variation or passive leg raising
  • MAP of >65 aim for higher if still signs of hypoperfusion or previous hypertension
  • Urine output to be maintained at >0.5

Discuss use of pressors in sepsis

A

Norad is the agent of choice

Dobutamine is the inotrope of choice in low cardiac output states but needs to be given with a pressor

Discuss adjunctive therapy

A

Corticosteroids – short ACTH stimulation test and serum cortisol levels are in no value in determining who should receive steroids. Remains controversial but is an agent that should be considered if refractory to fluids and pressors
