Discuss the aims of sediation and analgesia
1) relieve anxiety
2) ensure appropiate level of analgesia
3) facilitate ETT
4) treatement of riased ICP
Discuss level of sedation
Too deep sedation leads to sedation assocaited side effects , delayed weaning and possibly contribute to withdrawals
To light we lead to poor compliance with venilation, patietint distress and removal of lines, self extubation and other critical incidents
Neuromusuclar blockers may be needed to improve ventilation – especially in the early phase of ventilation when airway reflexes are more active
If using neuromuscular blocker infusion should be ceased 2x a day to check for agitation. This should be treated with agents like haloperidol rather then supressed
Discuss dose of propofol for induction
1-1.5mg/kg
