A
- rapport
- observation

Levels of Risk: Low Risk
A
- Fleeting thoughts of suicide,
- no plan, minimal/no substance misuse, stable environment
- no availability of means
Action:
• Diffuse emotional distress
• Lower stress demands
• Increase mental health interventions
• Monitor mental state as appropriate

Levels of Risk: Medium Risk
A
- Discrete suicidal ideation
- vague/no specific plans,
- some likelihood of rescue or intervention.
- Passive precautions (door unlocked)
- significant mental illness, substance misuse, unstable environment, dangerous behaviour
Action
• Diffuse emotional distress
• Involve significant others immediately
• Safety contract
• Once safety is obtained, full assessment & daily monitoring required, crisis management plans, intensive interventions
• Prevention strategies
• Coping strategies

Levels of Risk: High Risk
A
- Intense thoughts of death, seem impossible to get rid of.
- Definite suicide plan, significant mental illness, significant substance misuse, unstable environment, escalating dangerous behaviour
- Has a imminent date/time in mind. Clear threats
Action
• Ensure immediate safety (voluntary/involuntary admission)
• Immediate communication with relevant personnel
• Attend to own safety
• Where safety can be guaranteed, full assessment, crisis management, intensive interventions (regular contact)
