A 14 month old boy is brought to the ER by ambulance. He had been unwell for the last day with a sore ear. Mom had heard an odd sound coming from his bedroom and found him having a convulsion. The seizure lasted approx. 5 min. EMS reports that he had a temperature of 38 and that his BGL was normal. When you see him, he is initially drowsy but within 30 minutes begins playing with toys. Does this child require an LP?

No. In this case, suspicions are high for a simple febrile seizure with a focus for the fever (query otitis media). No indication for further investigations.

An 18 month old girls is brought to the ER. She tripped at home and fell. She cried immediately, but then “lost her breath” and became grey for 30 seconds. Her father gave her mouth to mouth and she came around almost immediately, becoming her normal self within 30 seconds. What is the most likely diagnosis? What would you want to rule out?

Most likely diagnosis: breath holding spell

R/O: cardiac cause (cyanotic heard disease)
