This mom should be provided reassurance. It is normal for babies to lose 10% of body weight in the first few days of life. By day 7-10 the weight should be back up to birth weight.

A = appearance/color (0 is pale or blue, 2 is pink body and extremities)
P = Puse/HR (0 is absent, greater than 100 is 2)
G = Grimace/Reflexes (0 is absent, 2 is cough, sneeze or pulls away)
A = Activity/Tone (0 is absent, 2 is active and spontaneous movement of limbs)
R = Respirations (0 is absent, 2 is good breathing with crying) 

Less than or equal to 3 requires resuscitation. Most babies are 8-9 and hardly ever 10.
