What are the two sets of acronyms you use during the secondary assessment?
SAMPLE & OPQRST
What does the S stand for in SAMPLE?
Signs/symptoms: what is causing you discomfort?
What does the A stand for in SAMPLE?
Allergies: are you allergic to anything? Exposed?
What does the M stand for in SAMPLE?
Medications: take any medications? Wrong dose? New meds?
What does the P stand for in SAMPLE?
Past medical history: past conditions, or incidents? Example diabetes
What does the L stand for in SAMPLE?
Last oral intake: what & when last eat/drink
What does the E stand for in SAMPLE?
Events leading up: what were you doing when it started?
What does O stand for in OPQRST?
Onset: did this happen suddenly? Or gradually?
What does P stand for in OPQRST?
Provokes: what makes the pain better or worse?
What does Q stand for in OPQRST?
Quality: type of pain? Squeezing, sharp, dull, throbbing?
What does the R stand for in OPQRST?
Radiating: is the pain “moving at all” to other areas?
What does the S stand for in OPQRST?
Severity: how severe on a scale of 1-10?
What does the T stand for in OPQRST?
Timing: when did it start? Constant or come and go?

What is the 3rd step in secondary survey?
Head to Toe examination
What are the three things you’re checking for during head to toe examination?
Skin
Palpate (slowly feeling for injuries)
Distal extremities
What are you checking for during skin on head to toe examination?
Color
Feel
Moisture
Temperature (use a thermometer when core temp relevant)
What are you checking for during Palpate in head to toe examination?
Slowly, thoroughly and methodically feel for injuries
What are you checking for in distal extremities of head to toe examination?
Compare pulses to other side
Compare capillary refill to other side
Check for motor deficits (wiggle your toes)
Check for sensory deficits (do u feel that)
