Scrotal Mass Flashcards
Which scrotal masses transluminates
Hydrocele
Epididymal cyst
(Fluid lets light shine through)
Which scrotal masses do not transluminate
Tumour
Orchitis
Haematocele
Granuloma
Varicocele
Epididymitis
Which scrotal swelling becomes larger when baby cries
Coommunicating hydrocele
What is the painless scrotal swelling that transluminates
Primary hydrocele
Which special investigations would you use to differentiate between epididymal cyst and hydrocele
US
Differentiate between torsion of the testes and torsion of epididymis
Acute vs gradual onset
Severe vs moderate pain
Associated trauma vs less likely (sexual hx)
Cremaster is reflex always absent on affected side vs intact cremasteric
Negative Prehn sign vs positive Prehn sign
What manoeuvre makes a vericicele detectable
Valsava manoeuvre
What are the management options for a varicocele
- Open surgical ligation
- Spermatic vein embolisation
a) anterograde sclerotherapy : sclerosant injected to cause fibrotic occlusion
b) Retrograde embolisation: catheter in left spermatic vein to IVC and left renal vein
c) Laparoscopy: clipping intraperitonealy
What are the features of TB epididymitis
Painless
Craggy non tender epididymal mass with normal testes
May be associated sinus onto scrotal skin
Thickened or beaded vascular diferens
On DRE: palpable seminal vesicles and hard prostate
What is the standard treatment for TB epididymitis
6 months TB treatment
How would you manage testicular torsion
NB it’s an emergency!!!
- Manual detorsion
-relieves pain and buys you time for surgery (will need surgery vele),so twist laterally, successful if marked relief - Then Emergency surgery
-Scrotal excision then detorsion -If viable orchidopexy (3 non absorbable sutures), if ischemic remove testicle (orchidectomy)
Contralateral orchidopexy in all cases - so always fix the other testicle so it doesn’t suffer the same thing
From which duct does the appendix testes come from
Mullerian (paramesomephrict) duct
From which duct does the appendix epididymis come from
Wolffian (mesonephric) duct
Which scrotal condition presents with a blue dot sign
Torsion of testicular or epididymal appendages (appendix)
What is the standard treatment for torsion of testicular or epididymal appendages
- Conservative
- If unsure, surgical exploration and excision of torted structure
- No need for contralateral exploration
