What is psychosis?
Represents an inability to distinguish between symptoms of delusion, hallucination and disordered thinking from reality.
What are positive symptoms of schizophrenia?
Hallucinations
Delusions
Disordered thinking
What are negative symptoms of schizophrenia?
Apathy
Lack of interest
Lack of emotion
Avolition - want to do tasks but can’t, suggests a poorer prognosis.
How long do you have to have the symptoms to meet diagnostic criteria of schizophrenia?
More than one month
What are the first rank symptoms of schizophrenia?
Alienation of thought
Delusions of control
Hallucinatory voices
Delusions (persistent)
What are the second rank symptoms of schizophrenia
Trains of thought
Persistent hallucinations
Catatonic behaviour: excitement or stupor
Negative symptoms
How many of each first and second rank symptoms do you need?
At least one first rank and two second rank.
What are the different kinds of psychotic disorders?
Delirium or acute organic brain syndrome
Affective psychoses
Manic episode
Schizoaffective disorder
What are the causes and symtoms of delirium or acute organic brain syndrome?
Brain/systemic disease
Visual hallucinations and illusions
Terror
Delusions are persecutory and evanescent
Worse at night
If a patient has a depressive episodes with delusions of worthlessness and derogatory auditory hallucinations, what psychoses do they have?
Affective psychoses
If a patient has delusions of grandeur, believing they are the Messiah, and show gross overactivitys, what psychotic episode do they have?
Manic episode
What is schizoaffective disorder?
Mix of affective and schizophrenia like symptoms
What is the prognosis for schizophrenia and psychotic disorder prognosis?
Up to 50% have a moderate recovery but there are a small group with chronic symptoms and little recovery.
