Prevelence of Schizophrenia in
1. General population
2. Single parent+
3. First degree relative
4. Dizygotic twins
5. Monozygotic twins
6. two parents+
7. Second degree relative

  3. First degree relative - 12%
  4. Dizygotic twins - 12%
  5. Monozygotic twins - 47%
  6. two parents+ - 40%
  7. Second degree relative - 6%

Diagnostic criteria for Schizophrenia

A

2 or more present
At least 1 present in first 3 points
For 6 months

  1. Delusions
  2. Hallucinations
  3. Disorganised speech
  4. Catatonic behaviour (disorganised)
  5. Negative symptoms

Mx of Schizophrenia

A

First line - Atypical antipsychotics
Olanzepine 5mg (or)
Risperidone 0.5mg

If not improved in 3wks ↑Dose
If not improved in 4-6 wks
1. Change drug same class (or)
2. Typical antipsychotic - Haloperidol/Chlorpromazine/Trifluperazine

If necessary - IV Haloperidol/Olanzepine

Preventing relapse of Schizophrenia

A
  1. Olanzepine
  2. Risperidone
  3. Quetiapine
    as low dose as possible

Drug resistant Schizophrenia

A

Cause could be drug abusers
Schizophrenia not controlled even with 3 different antipsychotics

Trial with Clozapine (S/E blood dyscrasias, Cardiotoxicity) or olanzepine

Consider ECT

  1. 1-30days : Brief psychotic disorder
  2. <6 months: Schizophreniform disorder
  3. > 6 months: Schizophrenia

Rx: Antipsychotics

Schizoaffective disorder, Mx

A

Schizo (psychosis) min 2 weeks
+
Bipolar (mania, hypomania, depression)

Antidepressants +/- mood stabilisers
Add Antipsychotics if not controlled by above

A

Late onset Schizophrenia
>60yrs, F
Paranoid psychosis
Rx- Risperidone/Olanzepine
