7. Appetite loss
8. Psychomotor changes
9. Suicidal
PDD Persistant depressive disorder definition, also called as?
Dysthymia
Mild, prolonged depression for 2yrs
Which c/f shows worst prognosis in MDD
Psychotic features
hallusinations etc
C/F, Mx of depression in children
Irritability
Difficulty sleeping
Not enjoying meals
Low self esteem
low concentration
MX:
Fluoxamine»
Fluoxatime
Recurrent brief depression, Mx
3-7 days episodes every month
Mx: CBT(Psychotherapy done by psychologist), Lithium
Seasonal affective disorder
Winter blues
Mx: CBT, Phototherapy, SSRIs if necessary
Mx of depression
Mild - CBt
Moderate - CBT +/- Antidepressants
Severe - CBT + Antidepressants, ECT(last resort)
SSRI takes 4-6wks for action
Continue for
1yr - first episode
2-3yrs - subsequent episodes
Seretonin syndrome symptoms
Mental, Muscle (skeletal and smooth)
1. Mental/behaviour changes
Hypomania, seizures, agitation,confusion
2. Skeletal muscles (LL>UL)
Tremors, myoclonus, hyper-reflexia
3. Smooth muscles/autonomic function
Diarrhoea, fever, tachy,
Hyper/Hypotension
- Diaphoresis (↑Sweating)
- Mydriasis
Drugs causing seretonin syndrome
Causing ↑Seretonin
1. Antidepressants
2. Selegiline
3. St Johns wort
4. Opioids (tramadol)
5. Lithium
6. Anti emetics
7. Illicit drugs
Seretonin syndrome vs Neuroleptic malignant syndrome
SS:
1. Acute onset, Rapid recovery
2. Hyper-reflexia
3. Myoclonus, tremors
4. Pupils - dilated
NMS - caused d/t antipsychotics
1. Gradual onset, prolonged recovery
2. Hyporeflexia
3. Diffuse RIGIDITY
4. Pupils normal
