1. Brief description of building or area of ops
2. Assignment/duties of members injured
3. Events leading up to injuries
4. What tasks injured members we’re performing
5. Suggestions to prevent similar injuries

If officer is on leave who fills out report

Answer 3

He does when he gets back

If leave more than 30 days then regular assigned officer will complete

If the company commander is not the officer on duty at the time of the multiple injuries he does not have to do anything for report

Answer 4

Must review report and add meaningful comments and suggestions that will help provide for future safety

Question 5

After reviewing report the company commander will forward report too

Answer 5

Chief of ops via chain of command

If no regularly assigned company commander than battalion commander will designate officer
