What is Sarcoidosis?
Formation of Inflammatory Nodules (Granulomas) across organs e.g. Lungs.
Distinguishing signs of Sarcoidosis?
Lofgren’s Syndrome
1) Erythema Nodosum (red, tender shins)
2) Hilar lymphadenopathy
3) Polyarthralgia - painful joints
Other organ signs?
Liver: cirrhosis + cholestasis
Heart: bundle branch block, heart block
Eyes: Uveitis, optic neuritis + conjunctivitis
Investigating suspected Sarcoidosis?
1) Bedside: ACE, IL2, CRP all raised
2) CXR + High Res CT: Pulmonary nodules + Hilar lymphadenopathy
3) PET scan: active inflammation
Gold Standard Diagnostic Test + signs?
Histology/Biopsy: No-caseating Granulomas visible
Management of Mild Sarcoidosis?
None
Management of Severe Sarcoidosis?
1st line: Bisphosphonates + oral steroids 6-24 months
2nd line: Methotrexate OR azathioprine
Lung Transplant if needed
Most common site of pulmonary fibrosis
Upper Lung Zones
