Findings in Sarcoidosis
- Erythema nodosum (painful papules), lupus pernio
- Symetric swelling of knees, PIP,MCP joints
- Female, young
- Multi-system involvement, lung most common
What is Sarcoidosis
Non caseating granulomas, multi-system
Cause unknown
Lofgren syndrome
Sarcoid syndrome
1. Erythema nodosum
2. BHL
3. Arthritis
Heerfordt-Waldenstrom Syndrome
Sarcoid syndrome
1. Uveitis
2. Parotid swelling
3. Facial palsy
CXR findings, 4 stages of sarcoidosis
- BHL
- BHL + PF
- PF alone
- Honeycombing fibrosis
Vit D, ACE levels increased/decreased in sarcoidosis
Vit D increased causing hypercalcemia
ACE increased - monitoring sarcoidosis
Diagnosis of sarcoidosis
CXR … CT… Biopsy
Confirmatory is lung/erythema nodosum skin biopsy showing non-caseating granuloma
Spirometry shows which pattern in sarcoidosis
Restrictive
DLCO decreased
Rx for hilar lymphadenopathy without lung involvement in sarcoidosis
Not required
Rx in sarcoidosis is required in which cases
- No improvement in 3-6 months
- Erythema nodosum + arthralgia
- Eye, CNS or other system involvement
- Hypercalcemia
- Symptomatic pulmonary lesions
Sarcoidosis is treated with which drug
