In regards to automatic recording systems what does the flight data recorder do?
it measures what the plane did/ compares the airplane to normal
What does the cockpit voice recorder do?
it measures what the pilots did
What is the NTSB definition of an Incident?
an occurrence other than an accident associated with the operation of an aircraft which effects safety of operations. Not all incidents need to be reported.
Examples of a reportable incident:
Damage to property more than 25,000 of fair market value
in flight fire
flight control system malfunction
complete loss of information for more than 50% of an aircrafts’s display
Internal incident reporting:
incidents are often precursors to accidents
reduce incidents, reduce accidents
look for correlations
What is the purpose of an Aviation Safety Action Program (ASAP)?
encourage employees to voluntarily report safety information
What employee groups are covered in the ASAP?
Pilots, Flight Attendants, Dispatchers, Airport Operations, Mechanics
Where does the ASAP report go?
Management, employee labor association, Faa Inspector
What is Employee Review Committee (ERC)?
this consists of representatives from different groups involved in the program
Why would you self report?
Determine whether such reports qualify for inclusion
identify actual or potential problems
propose solutions for those problems

What are some possible purposes for a safety committee?
review all incident reports
