it measures what the plane did/ compares the airplane to normal

it measures what the pilots did
What is the NTSB definition of an Incident?
an occurrence other than an accident associated with the operation of an aircraft which effects safety of operations. Not all incidents need to be reported.
Examples of a reportable incident:
Damage to property more than 25,000 of fair market value
in flight fire
flight control system malfunction
complete loss of information for more than 50% of an aircrafts’s display
Internal incident reporting:
incidents are often precursors to accidents
reduce incidents, reduce accidents
look for correlations
What is the purpose of an Aviation Safety Action Program (ASAP)?
encourage employees to voluntarily report safety information
What employee groups are covered in the ASAP?
Pilots, Flight Attendants, Dispatchers, Airport Operations, Mechanics

What is Employee Review Committee (ERC)?
this consists of representatives from different groups involved in the program

Rate is number of events divided by exposure to that event

whether the program’s total benefits to society exceed the costs
Cost Effectiveness Analysis (CEA)
you know the outcome, you want to determine which alternative programs achieve that outcome at the lowest cost

divide the monthly events by the months exposure
What is cumulative rates?
keep adding the events and exposures
What is moving average rates?
add the new events and exposure and drop the oldest
What are some safety budget items?
travel, safety seminars/ conventions, safety awards
What is selective neglect?
Look for high workload items with low payoff and neglect these items
