Reach rod was being operated; indication at VPI showed it was closed. The sheer pin had broke and the valve was actually open. You must look at the valve stem to see if the valve is in the position you are required to put it in.

We had voids, places where air was entrenched in our lines. The NRC thought we need to have the RAS line filled. We argued that our line was “self-venting”. Come to find out it was not self venting and we now fill the RAS line.
