Hendrich II Fall Risk Model
Assesses for confusion, depression, Altered elimination, Dizziness, Gender ( Male ), Medication complications, ability to stand: Score of 5 or higher indicates greater risk
Identify strategies to reduce the risk/prevent falls
- Check enviroment for saftey risks like clutterness
- Assistive devices like gait belt, wheel chair, walker
- Keep all doctor appoinements for patients
- Adequate nutrtion / hydration
- Assess patient, before moving to bed
- Assess envirement before moving a patient that has fallen.
- Grippy socks
- Handrails
- No bending at waist, more than 90 degrees
- Keep room light
What safety measures should be taken after a patient falls?

What are some safety considerations for transferring patients?
- Grippy socks
- Assistive devices ( Gait belt, walker, cane )
- Providers should avoid using their weight to lift patients.
- Providers should stay close to patients at all times during the transfer to keep the patient’s weight close to the provider’s center of gravity.
- Be careful not to let patients wrap their arms around the provider’s head.
