Gbs prophylaxis
- Benzylpenicillin 60mg/kg/dose bd
- grntamycin 2.5mg/kg 3/7
>30wks - daily
<30wks - q36hrs
Pertussis whopping cough
ANTIBIOTICS (if <14/7 cough) Neonate - Azithromycin 5/7 Child able to swallow - clarithromycin 7/7 VACCINATION - children and adults STAY HOME - daycare/school for 5-7 Days or 21 days after cough D/C criteria

Sepsis - suspected infants
3mo:
Cefotaxime 50mg/kg/dose QID 7days
mild croup Rx
CRITERIA:
- BARKING COUGH. no stridor at rest. no resp distress at rest. no oxygen req. no cyanosis
TREATMENT:
- 0.15-0.3mg/kg PO dexamethasone SINGLE DOSE
- D/C.
D/C criteria for croup
suitable if over the preceding hour:
- saturating >94% oxygen
- no stridor at rest
- no signs of resp distress at rest e.g.) recession
moderate croup Rx
CRITERIA: - stridor and respi distress at rest - no cyanosis. no oxygen req TREATMENT: - consider neb adrenaline: 0.5ml/kg (max 6ml) \+ - PO dexamethasone 0.15-0.3mg/kg OR - neb budesonide 2mg (if vom dex)
severe croup Rx
CRITERIA:
- severe respiratory distress; severe fatigue, alteration mental status. oxygen req
TREATMENT:
1) Oxygen via face mask (if ICU?
2) continuous nebulized adrenaline 0.5ml/kg
3) PO prednisolone .15-.3mg/kg OR
IV hydrocortisone
Croup d/c mx and advice to parents
Parents:
1) cough and low grade fever may cont for sev days (viral)
2) represent if stridor recurs, worsening resp distress –> sig airway obstruction. need immediate r/v
3) poor feeding or parental concern -> return
