RSV Flashcards
Describe the structure of RSV
Enveloped virus
-ve ssRNA non-segmented
What does the F-protein bind to trigger fusion
nucleolin
Where does RSV replicate
The cytoplasm
How many genes are on RSV
10 genes - each gene encodes a single protein except the M2 gene
11 proteins in total!
What direction does the polymerase move into make the mRNA
3’ to 5’ direction
Does splicing occur for RSV
NO as it replicates in the cytoplasm
How long is the incubation period
4 to 5 days
Who is most at risk fo getting it
those aged 6 weeks to 9 months
What are Ab produced against
The G and F proteins –> capable of preveting reinification
What proteins does the T lymphocyte respond to
N, SH, F, NS2, M2
What IL does subgroup A make to be more pathogenic
IL-13
How long is protein G
298 a.a.
The most variable gene product in any human infecting virus
What % of adults are reinfected by RSV
5-10% of health adults per year!
What motif in the G protein was thought to cause the enhanced disease in the formalin inactivated vaccine
The CXC3 motif in the centre of the G protein
It is a conserved region found between the 2 mucin domains
What does the CXC3 motif emulate
Fractalkine –> binds to certain neuro and microglia causing excitation and realease of products e.g. Substance P
What is the only licensed antiviral for RSV
Ribavain - a nuceloside analogue causing error catastrophes in mitosis
What is the new drug that can target the RSV polymerase
ALS-8176
Also some canditates to inhibit fusion
What is the mAB that targets the F protein to prevent fusion
Palivizumab
Who was Palivizumab given too
Those born <35 weeks
Those with chronic lung disease
Less than 6 months at the start of RSV season
What reduction in hospitalisation did Palivizumab give
55% reduction and decrease in stay duration
What is the cost of palivizumba
£3000 per patient
What is the newer version of palivizimab
Motavisumab –> had enhanced antigen binding and neutralisation stability
HOWEVER it wasnt licencsed by the FDA due to hypersensitivity reactions
What vaccine has given hope for an RSV vaccine
The rotavirus vaccine
As it face many of the same vaccine hurdles as RSV i.e. immunising very young etc.
