Cardiac Arrest
Adrenaline - 1mg IV, 1 in 10,000, every 3 - 5 minutes
Amiodarone - initially 300mg IV, in 20ml of Glucose 5%
- further 150mg IV if required
- followed by 900mg IV infusion over 24hrs
Acute Pulmonary Oedema
Furosemide - 40-80mg, IV
Oxygen - 15L via NRBM
Morphine - 5 - 10mg, IV (or diamorphine 2.5 - 5mg IV)
Nitrates - GTN spray - 800mcg Buccal spray
- followed by 10mcg/minute infusion
Acute severe asthma
Salbutamol - 5mg Neb, back2back (every 20-30mins)
Ipratropium - 500mcg, Neb, every 4 hours
Steroids - Hydrocortisone 100mg IV
- Prednisolone 40-50mg, PO
Magnesium sulphate - 1.2 - 2g, IV, over 20 minutes
Oxygen - 15L, via NRBM
Anaphylactic shock
Adrenaline - 500mcg, 1 in 1000, i.e. 0.5ml
Hydrocortisone - 200mg slow IV, or IM
Chlorphenamine - 10mg slow IV, or IM
Fluid challenge - 500 - 1000mL, crystalloid STAT
Benzodiazepine intoxication
Flumazenil - 0.2mg, IV over 30 seconds
- 0.3mg, after 30 seconds
- 0.5mg, at 1 minute intervals
Max dose 2mg
Hypoglycaemia
Glucose - 10g, 20% glucose IV infusion
Glucagon - 1mg, SC or IM
Hypovolaemic shock
Fluids - 10-15mL/Kg - crystalloid (Hartmann’s or Saline)
If no improvement after 2 boluses, ICU referral
After 2L of fluid, blood transfusion
Opioid intoxication
Naloxone - 0.8mg IV, repeated at 2-3 minute intervals
Seizures
Lorazepam - 0.1mg/Kg, IV - 2g/minute - maximum 4mg
Diazepam - 0.2 - 0.5mg/Kg - rectally
Phenytoin - 20mg/Kg loading dose + 5-10mg/Kg if necessary
Hyperkalaemia
Calcium gluconate - 30mls of 10%, IV
OR Calcium chloride - 10mls of 10% IV
Insulin glucose solution -
25g Glucose - 50ml of 50% OR 125mls of 20%
Insulin 10 units of Actrapid
Salbutamol - 10-20mg Neb
Calcium resonium - 15mg QDS PO
OR - 30mg QDS PR
