Risk of major bleeding for bleeding for patient on anticoagulation to asses risk in af care.
- Hypertension – uncontrolled >160
- Renal disease – dialysis, creatinine >2.26 or >200 umol/L
- Liver disease – cirrhosis? - bilirubin over 2* normal – ast/alt/ap >3* normal
- Stroke history
- Prior major bleeding or predisposition to bleeding
- Labile inr – unstable/high inr - <60%
- Age >65
- Medication predisposing to bleeding – aspirin, clopidogrel, nsaids
- Alcohol - >8 drinks/week
