Rheumatology: Vasculitis Flashcards
What are the 3 types of Vasculitis?
Large, medium and small
What are the subtypes of LARGE vessel vasculitis?
- Giant Cell Arteritis
- Takayasu’s
“it TAKes a Giant to be LARGE”
What are the subtypes of MEDIUM vessel vasculitis?
- Polyarteritis Nodosa
2. Kawasaki’s Disease
How are small vessel vasculitises categorized?
ANCA + or ANCA -
ANCA= AntiNeutrophil Cytoplasmic Antibody
What are the subtypes of ANCA + small-vessel vasculitis?
- Granulomatosis w/polyangiitis (GPA or Wegener)
- Eosinophilic granulomatosis with polyangiitis (EGPA or Churg-Strauss)
- Microscopic Polyangiitis (MPA)
What are the subtypes of ANCA - small-vessel vasculitis?
- Henoch-Schönlein Purpura (HSP)
- Essential cryoglobulinemic vasculitis
- Cutaneous leukocytoclastic angiitis
When I say Giant Cell Arteritis, you think:
- Temporal Headache
- Jaw Cramps.
- Intima fibrosis
When I say Takayasu’s, you think:
- Aortic arch
2. Cramping or weakened pulse of upper extremeties
When I say Polyarteritis Nodosa, you think:
Panmural necrotizing arteritis
When I say Kawasaki’s, you think:
Motorcycles….
Wouldn’t it be fun to be dirt biking right now? We gotta go to my cabin in Westcliffe soon and bring all our LITTLE ASIAN BABIES with us.
Think of all the little Asian babies…
When I say Churg-Strauss, you think:
- EOSINOPHILIA
- Necrotizing extravascular granulomas.
ANCA+
When I say Microscopic polyangiitis you think:
- Palpable purpura
- Joint pain
ANCA +
When I say Henoch-Schönlein Purpura (HSP), you think:
IgA deposition
ANCA -
When I say Essential cryoglobulinemic vasculitis, you think:
WTF is that?
- Immunoglobulins precipitate due to cold.
- Hep C +
- ANCA -
c-ANCA binds to
PR3 (proteinase 3) found in PMNs
p-ANCA binds to
MPO found in PMNs.
What are the immunopathologies of Vasculitis?
- Immune Complexes
- ANCA
- Antiendothelial ABs
- T Cell dependent endothelial damage
- Infection of endothelium.
Basically, anything that will damage/irritate a blood vessel. I mean, it makes sense right?
T/F
In primary vasculitis, the inciting agent is unknown
True, straight from the PDF.
How do you treat Vasculitis?
Treat or remove the inciting agent or antigen.
Also, glucocorticoids help.
