AP is notified of the tasks to be performed

AP agrees with modified work and commute is within the IW capabilities

The employer provides a written offer to the worker with the following
Start Date and Time
Duration of the Modified Work
Statement the AP has approved the work
Description of physical requirements
Place/location of work
Hours to be worked
Wages for the modified work

A

Employer has RTW policy

AP has approved the modified job description

Memo in file includes
     Hours and days to work
     wages for the modified work
     start date and time
     statement that modified work would be available if 
     worker had not been terminated
