Resuscitation Flashcards
Resuscitation Guidelines
Updates to Resuscitation
(6)
- interlinked system of care approach
- early advanced airway management
- induction of amiodarone for adult refractory cases
- first defibrillation is an AED mode
- resuscitation checklist
- clinical review and debrief
Resuscitation Guidelines
Principles of resuscitation
(7)
- high-quality compressions
- pad placement and replacement
-early airway management
-ventilation - interruptions under 5 seconds
- refined pharmacological prioritisation
- communication, planning & teamwork
Resuscitation Guidelines
Inadequate pulse
>1yrs old
40bpm
Resuscitation Guidelines
Inadequate pulse
<1yrs old
60bpm
Resuscitation Guidelines
Good Quality Compressions
4 factors
- 100 per minute
- depth 1/3 chest (minimum 5cm for adults)
- rotate every 2 minutes
- minimise interruptions <5seconds
Resuscitation Guidelines
Correct Defib Pad Adhesion
6 factors
- ensure safety (4 checks)
- dry skin, remove sweat/moisture
- shave hair if needed
- ensure no creases or bubbles in adhesion
- ensure in date and good gel
- replace after 3 shocks, consider alternate location
Resuscitation Guidelines
Correct Defib Pad Placement
antero-lateral
Sternal Pad - right of sternum below clavicle
Apex Pad - over 5th intercostal space, mid-axillary line (V6 lead)
Resuscitation Guidelines
Correct Defib Pad Placement
antero-posterior
Anterior - left side chest, below nipple/breast
Posterior - left of spine below scapula
Resuscitation Guidelines
Resus Airway Management
5 factors
- iGel placed early
- airway management does not interrupt compression
- once airway is in 10 breaths a minute, uninterrupted compressions
- good BVM technique, reduction in gastric insufflation
- low threshold for gastric decompresssion
Resuscitation Guidelines
4 H’s
- hypoxia
- hypo/hyperthermia
- hypo/hyperkalaemia
- H+ (acidosis)
Resuscitation Guidelines
4 T’s
- toxicity
- tension pneumothorax
- tamponade
- thrombus
Resuscitation Guidelines
Inadequate Perfusion
(4)
- unresponsive
- pallor or central cyanosis
- inadequate pulse
- first rhythm analysis in AED mode but must confirm if shockable
Resuscitation Guidelines
Minimising Interruptions
(4)
- plan ahead for shoulder-to-shoulder swap
- anticipate defib - charge at 1:45
- rapid rhythm analysis
- immediate restart of CPR post defib/check - hands above the chest
Resuscitation Guidelines
Adrenaline
Indications
(1)
- cardiac arrest
Resuscitation Guidelines
Adrenaline
Contraindications
(1)
- nil
