Restricted Airflow
“Restriction” in lung disorders always means a decrease in lung volume (FVC)
PE: Coarse Crackles
Asbestosis
Characterized by slowly progressive (years), diffuse pulmonary fibrosis
malignant mesothelioma!
DOE
If cough, sputum production, or wheezing are present more likely secondary to smoking
Bronchiolitis Obliterans (BO)
chronic airway rejection in lung transplant patients
Usually indolent symptoms similar to URI
DOE, decline in spirometry
Hypersensitivity Pneumonitis
Represents an immunologic reaction to an inhaled agent
Subacute HP: Acinar Nodules
Chronic HP: Fibrotic changes
Interstitial Lung Disease
diffuse parenchymal lung diseases
presents w/ progressive DOE & nonproductive cough
x-ray w/ haziness progresses to nodules then linear opacities
Idiopathic Pulmonary Fibrosis Treatment
Glucocoricoids
Immunosuppressives: Azathiprine, Cyclophosphamide, Methotrexate
Antioxidants: Acetylcysteine
Sarcoidosis
A multisystem granulomatous disorder of unknown etiology
It is characterized by the presence of noncaseating granulomas in involved organs
Typically affects young adults, initially presents w/ one or more of the following:
Bilateral hilar adenopathy
Pulmonary reticular opacities
Skin, joint and/or eye lesions
Radiation-induced Lung Injury PE
Fine crackles or a pleural rub, sometimes normal
Pleural friction rib
Dullness to percussion
Tachypnea, cyanosis or signs of pulmonary HTN (severe)
Pneumoconiosis
nonneoplastic reaction of the lung to inhaled mineral or organic dust
Silicosis and Coal workers
