Amyloidosis
Sarcoidosis
Scleroderma
Post MI fibrotic
Parthophysiology
Heart wall is rigid and has difficulty stretching, pumping
Ventricles restrict = decreased cardiac output
Bi-atrial enlargement
Impaired ventricle filling due to rigidity
Decreased cardiac output
Symptoms
Severe
Dyspnoea
S3 + s4 heart sounds
Oedema
Congestive HF
Signs
S3 + S4 heart sounds
Narrow pulse pressure =
- normally 120/180
DCM around 105/95 (there for blood stasis as decrease gradient)
Diagnosis
ECG
GOLD STANDARD = ECHO
Cardiac catheterisation
Treatment
Non - consider transplant
Treat cause.
Patient usually dies within a year
