Asthma

A

Mild- moderate:
pMDI 4-12 doses @ 20/60 until resolved
4 breaths per dose.

Severe or no improvement after 20/60:
Salbutamol 10mg neb and Ipatropium Bromide 500mcg neb

  • Repeat Salbutamol 5mg neb 5/60
  • dexamethesone 8mg IV/oral

Inadequate response:
Adrenaline 500mcg IM (1:1000)
Repeat 500mcg IM 5/60 Max 1.5mg

Unconscious
Ventilate 5-8 ventilations minute

Loss of cardiac output
1/60 apnea
Exclude TPT
If pulse doesn’t return mx as arrest

A

Upper airway obstruction

A

Conscious patient

  • encourage to cough
  • 5 back blows
  • 5 chest thrusts
  • alternate above options and monitor for deformation

Unconscious patient

  • laryngoscope and Magills
  • if obstruction remains- chest compression
  • if loss of cardiac output tx as arrest

All other strider

  • adrenaline 5mg nebulised
  • consult for repeat doses
  • notify hospital
  • Dexamethasone 8mg IV/oral
