presentation of croup
how does it start and evolve
starts with coryza and fever
then hoarseness - vocal cord inflammation barking cough (tracheal oedema harsh stridor recession etc
when does croup tend to start/get worse?
time of day
what does this mean?
at night.
so dont send people home if they have presented earlier
causes of croup
VIRAL
1 - parainfluenza
also rhinovirus, RSV, influenza
what age children are most likely to get croup?
6months - 6 years
peak at 2 years old (autumn)
what features indicate that croup is moderate/severe and hospital is needed?
stridor and recession.
if agitated/lethargy - severe
mild may only have barking cough
first line treatment of croup
dexamethasone PO
or prednisolone PO
or budesonide (neb)
every 12 hours if necessary
second line treatment of croup
adrenaline + O2 nebulised
this can wear off so observe for 2-3 hours
intubate if this does sort it
differentials for stridor and fever
croup
epiglottitis
bacterial trachealitis
what season is croup season?
autumn (september-december)
