Q

lung fissures and markings?
lung boundaries?
lobe marking differences?
parietal &visceral pleura markings?

A

oblique-join T3 to CC6
horisontal(R only)- CC4 follow rib to oblique at axillary

medial 3rd clavicle, sternoclavicular, CC 2,4,6, median, rib6 clavicular, rib8 axillary, rib 10 scapular/posterior median, T1 post median

horisontal-upper/middle
oblique-middle/lower(R) or upper/lower(L)
on L lung 6 CC 3cm lateral due to cardiac notch (and visceral but not parietal)

pleura same levels as lungs up to CC6(parasternal)
then
parietal- rib8 clavicular, rib 10 axillary, T12 scapular, L1 post median
visceral- rib 6 clavicular, rib 8 axillary, T12 scapular

Q

Describe & demonstrate the surface marking of mediastinal pleura of right & left lung on the anterior chest wall

A

Right: Parietal pleura projects above 1st costal cartilage, extends into neck above the medial 1/3rd of the clavicle. Runs down sternum to level of 6th costal cartilage.

Left: Parietal pleura projects above 1st costal cartilage, extends into neck above the medial 1/3rd of the clavicle. Runs down sternum to level of 4th costal cartilage. Deviates to left to allow for cardiac notch – 2-3cm.

Q

Describe & demonstrate the surface marking of the inferior margin of parietal pleura of right/left lung.

A

Right: Transverse process of T1 vertebrae, around root of neck, parietal pleura projects above 1st costal cartilage, extends into neck above the medial 1/3rd of the clavicle. Runs down sternum to level of 6th costal cartilage. Reflects onto the diaphragm above costal margin, goes around the thoracic wall following rib 8 (midclavicular line), rib 10 (midaxillary line), rib 12 (T12), transverse process of L1 vertebra.

Left: Transverse process of T1 vertebrae, around root of neck, parietal pleura projects above 1st costal cartilage, extends into neck above the medial 1/3rd of the clavicle. Runs down sternum to level of 4th costal cartilage. Deviates to left to allow for cardiac notch. Reflects onto the diaphragm above costal margin, goes around the thoracic wall following rib 8 (midclavicular line), rib 10 (midaxillary line), rib 12 (T12), transverse process of L1 vertebra.
