Initiating Basal-Bolus Insulin in Type 1 Diabetes
- Calculate total daily dose using TBW: 0.5 units/kg/day.
- Divide into 1/2 basal & 1/2 rapid-acting. If using NPH/R, use 2/3 & 1/3.
- Split rapid-acting among meals.
Insulin-to-Carbohydrate Ratio for Regular Insulin
450/TDD = grams of carbohydrate covered by 1 unit of regular insulin
TTD = total daily dose
Insulin-to-Carbohydrate Ratio for Rapid-Acting Insulin
500/TDD = grams of carbohydrate covered by 1 unit of rapid-acting insulin
TTD = total daily dose
Correction Factor for Regular Insulin
1500/TDD = correction factor for 1 unit of regular insulin
TTD = total daily dose
Correction Factor for Rapid-Acting Insulin
1800/TDD = correction factor for 1 unit of rapid-acting insulin

What are the exceptions for
Insulin Conversion
- NPH dosed BID → glargine dosed daily: use 80% of NPH dose
