When should insulin be used as the initial treatment for T2DM?
A1C > 10 or BG ≥ 300
How to initiate insulin in T2DM?
- Add basal insulin: 10 units SC daily or 0.1-0.2 units SC/kg/day
- Add prandial insulin (if FPG not at goal or signs that prandial insulin is needed): 4 units or 10% of basal dose SC QD prior to largest meal
- If A1C still not at goal → full basal/bolus regimen (basal insulin daily + prandial insulin before each meal) or mixed insulin regimen (NPH BID + short/rapid self-mixed or premixed)
