What can you do to treat secondary hypogonadism?
Treat with gonadotrophins (LH and FSH) > induces spermatogenesis.
LH stimulates Leydig cells to increase intratesticular testosterone to significantly higher levels than in circulation (X100).
FSH stimulates seminiferous tubule development and spermatogenesis.
Why would you avoid testosterone in individuals desiring fertility?
In individuals desiring fertility avoid testosterone administration because it exerts negative feedback on gonadotrophic release (LH and FSH release) > Further reduce spermatogenesis.
Secondary hypogonadism is associated with low sperm and testosterone levels, thus what is administered?
If no response occurs after 6 months what do you do?
hCG injections which behave one LH receptors.
No response after 6 months > FSH injections > directly induces spermatogenesis.
