What are the main distinctions between an IFSP and an ISP?
IFSP=birth to 3 years, includes family needs and focuses on special services needed
ISP=3 years or older, long-term care needs, focuses on adult individual, may include plan for housing, employment, medication, etc.
What are some key reasons why reports are important and necessary?
An ongoing record of services received (when providers change)
A resource for other service providers
Eligibility for social assistance and insurance claims
Save time; avoidance of duplicated efforts and/or wasting time with methods that have already been unsuccessfully attempted
