When do you need to write a report?
When you encounter anything unexpected or out of the ordinary, or when asked to do so.
Can you give some examples of events that would require you to write a report?
- Delays
- AWS/TPWS activations
- Faults
- Signal irregularities
- H&S incidents
- Fires
- Issues with the permanent way
- Injuries
- Passenger influenced incidents
- vandals and trespassers
- safety of the line incidents
Can you give some examples of forms that are used?
- RT3185
- Delay attribution form
- Pass com activation form
- General report form
- In cab CCTV defect form
- Passenger disruption form
- Train fault form
- General accident report form
How can reports be submitted and when must this be done?
Reports must be submitted to the RM at the end of your shift. They can be handed to the RM in person, scanned (or photographed) and emailed to the RM, Faxed, or given to a colleague to hand over on your behalf.
It’s a good idea to keep a copy for yourself.
When must the RT3185 be completed?
When a wrong sided failure is identified this needs to be completed immediately at the location. This includes for wrong sided TPWS and AWS failures, such as no horn or bell when approaching a stop aspect.
Right sided failures can be reported as soon as practicable. This could include an AWS horn for sounding at a proceed aspect for example.
