Clozapine
(1) Check patient’s ANC (absolute neutrophil count) as required
(2) Pharmacies must have representative oversee process and enroll in program, train staff, obtain authorization to dispense each prescription by using the REMS program website/call center, maintain records
(3) Patients must be provided with counseling and guide, be enrolled in program, get blood work
Isotretinoin
Get pregnancy tests, use 2 methods of contraception, not donate blood
Phentermine/topiramate
All patients receive education about birth defects, provide medication guide and risk of birth defects brochure for all patients, obtain authorization to dispense each prescription, not distribute/transfer/loan or sell drug
Oxycodone + Acetaminophen
(1) Training and education about recommended pain management practices and appropriate use of opioid analgesics.
(2) Patients must be informed about their roles and responsibilities regarding their pain treatment plan, including the risks of opioid analgesics and how to use and store them safely, as outlined in the Medication Guides and Patient Counseling Guide for opioid analgesics.
Codeine + Acetaminophen - Tylenol #2, Tylenol #3
(1) Training and education about recommended pain management practices and appropriate use of opioid analgesics.

Methadone - Dolophine

Buprenorphine
(Quiz) Prescribing is limited to providers who have met qualifications and received a specific DEA number (X-waiver). Some exemptions to this were issued recently.
(1) Healthcare providers must assess patient’s opioid dependence and progress toward treatment goals, complete appropriate use checklist, and prescribe a limited amount of medication
(2) Patients must receive counseling on risks and safe storage of medication, and be monitored for appropriate medication use
Buprenorphine/naloxone - Suboxone
(1) Providers who prescribe must assess patient’s opioid dependence and progress toward treatment goals as part of the REMS requirements
(2) Patients must receive counseling on risks and safe storage of medication, and be monitored for appropriate medication use
