Amiodarone
Congenital goiter/hypothyriodism and hyperthryroidism
Warfarin - Coumadin
Fetal hemorrhage and increased risk of spotaneous abortion
Phenytoin - Dilantin
Risk of cleft lips and cardiac defects in fetus
Phenobarbital
Crosses the placenta and may pose fetal harm
Use in last trimester may cause withdrawal symptoms in infants. Excreted into human breast milk; monitor infant for sedation
Lithium
May cauase fetal harm
Excreted into human breast milk - avoid use during breastfeeding
Clozapine
Other agents preferred in pregnancy
Health care providers are encouraged to enroll women 18 to 45 years of age exposed to clozapine during pregnancy in the Atypical Antipsychotics Pregnancy Registry
Not recommended for breastfeeding women due to side effects
Isotretinoin
Boxed warning for teratogenicity
Breastfeeding not recommended during treatment and for 1 month after last dose due to potential for serious adverse effects
Phentermine/topiramate
Increased risk of congenital malformations, specifically orofacial clefts, in infants exposed during 1st trimester
Not recommended for breastfeeding women due to side effects
Valproic acid
Boxed warning for causing congenital malformations, particularly neural tube defects (spina bifida)
Carbamazepine
Teratogenic effects, higher risk with higher doses and anticonvulsant polytherapy than monotherapy
Oxycodone + Acetaminophen
Boxed warning for neonatal opioid withdrawal syndrome if used during pregnancy
Codeine + Acetaminophen - Tylenol #2, Tylenol #3
Boxed warning for neonatal opioid withdrawal syndrome if used during pregnancy
Hydrocodone + Acetaminophen - Vicodin
Boxed warning for neonatal opioid withdrawal syndrome if used during pregnancy
Fentanyl - Duragesic
Boxed warning for neonatal opioid withdrawal syndrome if used during pregnancy
Oxycodone - OxyContin
